What happens to the information you send through this website, who handles it, how long it is kept, and what you can ask us to do about it.

Who is responsible for your information

Sherif Kamel is the data controller for information submitted through this website. He practises at Westdale Dental Practice, 406 Westdale Lane, Mapperley, Nottingham NG3 6DG, and at The Dental Suite, Melton Road, West Bridgford, Nottingham NG2 6EN, and is registered with the General Dental Council under number 233860.

What this notice covers

This notice covers the two forms on this website — the enquiry form and the referral form — and any email or phone contact that follows from them. Once you become a patient, the separate privacy notice held at the practice where you are treated governs your clinical record.

What is collected

  • Enquiry form Your name, email address, phone number if you give one, the subject of your enquiry, whether you are a referring clinician, and whatever you write in the message box.
  • Referral form The referrer’s name, practice, address, phone and email; and the patient’s name, date of birth, address, phone, email, the reason for referral and the clinical summary written by the referrer.
  • Server records The website itself sets no cookies and runs no analytics or advertising trackers. The company hosting the site keeps standard server logs, which include IP addresses.

If you are being referred by your dentist

Where a dentist or GP refers you, they send us your details rather than you sending them yourself. We receive your name, date of birth, contact details and a clinical summary from them. Your referrer should tell you they are making the referral. You have the same rights over that information as if you had sent it yourself, and this notice explains them below.

Why it is collected, and the legal basis

Information from the enquiry form is used to answer your enquiry and, where you ask for one, to arrange a consultation. Information from the referral form is used to assess and arrange the treatment your referrer has asked for, and to write back to them afterwards.

Under UK GDPR the lawful basis is Article 6(1)(f), legitimate interests — responding to someone who has contacted a dental practice about treatment. Health information, including anything clinical you or your referrer describe, is special category data, and the additional condition is Article 9(2)(h): the provision of health care and the management of health care services. Nothing collected here is used for marketing, and none of it is used for automated decision-making or profiling.

Who else sees it

  • Formspree Both forms are delivered by Formspree, Inc., a form-processing service based in the United States. Submissions pass through and are stored on their systems before reaching the practice inbox. They act as a processor under a data processing agreement and transfers are covered by the UK extension to the EU–US Data Privacy Framework and standard contractual clauses.
  • The practice you attend Westdale Dental Practice or The Dental Suite, so your appointment can be booked and your treatment recorded.
  • Your referring dentist or GP Where you were referred, they receive a written summary of the assessment and of any treatment carried out. This is a normal part of shared clinical care.
  • Google Maps The maps on the contact page are not loaded until you choose to load them. Nothing is sent to Google, and no Google cookie is set, unless you click.

Your information is never sold, and is never passed to anyone else for marketing. It is disclosed beyond the people above only where the law requires it, or where there is a serious risk to someone’s safety.

How long it is kept

An enquiry that does not lead to treatment is deleted within twelve months. A referral that does not lead to treatment is deleted within twelve months of the last contact about it. Where treatment goes ahead, the information becomes part of your clinical record and is kept for the period required of dental records — normally at least eleven years for an adult, or until a child reaches twenty-five, whichever is longer.

Your rights

You can ask for a copy of the information held about you, ask for it to be corrected if it is wrong, ask for it to be deleted where there is no longer a reason to keep it, ask that its use be restricted, or object to how it is being used. Requests are free and are answered within one month. Email info@oralsurgerynottingham.co.uk or write to either practice address above.

If you are not satisfied with the response, you can complain to the Information Commissioner’s Office at ico.org.uk/make-a-complaint or on 0303 123 1113. You are welcome to raise it with us first, but you do not have to.

Complaints about treatment

If you are unhappy with any aspect of your care, please say so — either to Sherif directly or to the practice where you were treated, who will acknowledge your complaint within three working days and explain how it will be looked into. If you would rather raise it elsewhere, private dental complaints can be taken to the Dental Complaints Service on 0345 120 5744, and concerns about a clinician’s registration to the General Dental Council.

Security

This website is served over an encrypted connection and holds no database of its own. Form submissions are protected in transit and are accessible only to the practice inbox. Ordinary email between different providers is not encrypted, which is why referrers are asked to send radiographs from a secure account where they have one.

Changes to this notice

This notice was last updated in September 2026. If it changes materially, the updated version will appear on this page.
